An exosome is a cellular byproduct that communicates with cells. Stem cells use exosomes to send signals to cells to tell them what to do.
This can be a bit confusing, so picture a stem cell with an oil film around it. Part of the film turns into a bubble, which is the exosome. The information the stem cell contains goes into the bubble, and the bubble then breaks away from the stem cell. It goes to a nearby cell, makes contact, and becomes part of it. Then, it transfers the information it has to the new cell, telling it what to do.
Keep in mind that exosomes aren’t just related to stem cells. Other cells, including bacteria and viruses, produce exosomes.
As you age, the number of exosomes your cells send decrease and your cells get older. However, exosomes may help to reduce the signs of aging. Assume you were to take stem cells from a 20-year-old and a 60-year-old. You place the stem cells on a table with a barrier between them, so they cannot touch. The exosomes would still communicate through that barrier, and the older stem cells would become younger. The cells show signs of reversing some of the characteristics of the aging process because that’s what the exosomes told them to do.
Researchers have discovered that exosomes are in control of the cellular environment. Through communication, exosomes implant the information needed to heal cells. The information packet might include microRNA and DNA.
Research is still being conducted on exosomes, however, studies have shown that exosomes create an anti-inflammatory response that can help people who suffer from autoimmune disease.
Researchers have also studied using exosomes for cosmetic treatments, such as microneedling. Practitioners create inflammation, and then the exosomes communicate with the cells during the healing process.
Exosomes are fast-acting. Researchers have discovered that once injected, exosomes produce an immediate reaction. The reaction lasts for approximately 24 hours. However, that doesn’t mean that people will see immediate results when undergoing exosome therapy. Exosomes insert microRNA into target cells. This RNA reprograms the cell, and that process can take up to eight weeks, although it might be months before people enjoy the results.

Stem cells multiply, causing people to wonder if exosomes do the same thing. While they can share information that causes other cells to multiply, exosomes do not multiply.
Stem cells and exosomes are evaluated at FDA-certified laboratories. During rigorous testing, the laboratory ensures that GMP-grade manufacturing methods were used when procuring the exosomes.
The surfaces of exosomes do not contain any immune markers. Because of this, it is very unlikely that a body would reject exosomes.
Because exosomes do not engraft into the DNA or multiply, they cannot cause tumors.
There are some signs that you can look for that indicate that the exosomes are repairing and regulating your cells. At first, you should notice fewer inflammatory symptoms. Then, over the course of a few weeks, you might experience fluctuating energy and flulike symptoms. At 6-8 weeks, the second phase of the repair process usually begins, so those symptoms will likely go away. Once this phase is complete, you could notice an improvement. Note: You may see competitor marketing materials that state “products of conception” do not contain live cells. This is true if the biologics are radiated during processing or contain too much preservative. But it is not true if the materials have been processed without significant radiation or preservatives. Absolutely. Countless pain management and orthopedic practices have embraced regenerative medicine.
No they are not. They are not regulated as drugs, rather they fall into the biologics category. These are regulated heavily by the FDA, but do not get approved or denied.Here is how the FDA regulates things:
Medical devices – joint implants, screws/rods, DME, etc. In the world of regenerative medicine, this only applies to the kits used in PRP, bone marrow or adipose procedures.
Drugs – think of Lipitor, Vicodin, Viagra, etc. These are medications that have gone through a clinical trial and been approved for a specific indication.
Biologics – the FDA strictly regulates how biologic materials are acquired, processed, stored and used under the CFR Part 1271. Amniotic and umbilical materials fall under this category, which does not involve an Approval/Denial process like drugs do.
Specifically, the section under Part 1271 that applies to amniotic/umbilical tissues is Section 361 products, which are not required to be licensed or approved by the FDA and are regulated under Section 361 of the Public Health Service (PHS) Act.
Theoretically the answer is yes. However, it’s currently not clinically relevant. Embryonic stem cells are pluripotent and can differentiate into over 200 cell types.
However, there are ethical concerns over how they are obtained, and also the cells themselves often do not know when to stop replicating. This can form tumors.
The non-embryonic adult mesenchymal stem cells obtained from one’s bone marrow, amniotic fluid or umbilical cord can still differentiate into several important cell types, but not 200.
This is called being Multipotent and able to turn into cartilage, muscle, tendon,ligament and most vital organ cells. Multipotent stem cells have not been shown to form tumors like the pluripotent variety.
Procedures available for stem cell therapy include localized or IV injection with amniotic or umbilical cord derived stem cell therapy.
The procedures are all performed as an outpatient, with each procedure taking less than an hour. With the amniotic or umbilical cord therapy, the fluid is obtained from consenting donors after a scheduled C-section. Then it is processed at an FDA regulated lab and cryogenically frozen and thawed out right before use.
Clinics use regenerative medicine to treat a variety of conditions, including arthritis, tendonitis, cartilage defects, diabetic neuropathy, fractures, hair loss and aging. However, researchers have conducted several small studies with animals and humans as subjects. The results of these studies have been positive. The results support the hypothesis that regenerative medicine is key to improving musculoskeletal conditions.
